Template treatment plan
Patients and health professionals can use our template treatment plan to support self-management of LS.
It comes with clear diagrams and prompts including information about your treatment, how often you should use it, how much to use it each time, and where to apply it. All of which is designed to be tailored specifically to you, with the help of your health professional.
This is designed to be a memory aid, so that when you leave your appointment you leave with all the information you need to feel confident managing your LS at home.
Checklist of symptoms and concerns
We’ve designed this with patients and expert health professionals to be a useful document for people who think they might have LS and want to raise it with their health professional (for example a GP). And for people who already have a diagnosis, it might useful to take to your check-ups or other appointments.
There’s even a diagram to help you identify where your symptoms are happening, and a list of possible concerns you might want to raise with your health professional.
